Statement of Financial Responsibility
Your medical provider is sending your specimen to Scientia Diagnostics, LLC (“SDx”) where highly trained clinical laboratory scientists will evaluate your specimen utilizing state-of-the-art laboratory instruments. All professional services rendered are charged to the patient. Necessary forms are needed to help expedite insurance provider payments, however, the patient is responsible for all fees. SDx will send a statement after your insurance provider has processed the claim and determined your deductible, co-payment and or co-insurance amount.
Medicare and Medicaid
SDx is an in-network provider for traditional Medicare part B. SDx will file a claim on your behalf to your insurance plan. You will be responsible for any deductible, co-payment and or co-insurance amount.
Third Party Insurance Provider
SDx accepts payment from many insurance companies and will file a claim on your behalf. SDx reserves the right to reject unacceptable settlement offers from non-contracted insurance plans. SDx will bill patients for insurance deductibles, co-payments and co-insurance amounts determined by the insurance provider. SDx will work with patients on an individual basis to establish payment options on any outstanding balances through the Financial Assistance Program. All invoices include a billing phone number for patients t contact SDx directly with questions or concerns. Uninsured patients will be billed directly at special rates.
